Make atm WARNS=6 clean. The changes are mostly:
- remove some instances of __P()
- use real prototypes and un-K&R function headers
- constify where necessary (mostly strings and structures containing
strings)
- make functions and variables static that need not to be global
- tag unused function parameters as __unused
Testing: a fresh universe
