Passes basic pthread_mutex and PI tests out of ltp/testcases/realtime. The mmap_sem is no longer held outside of the futex_handle_fault() routine, yet numerous comments refer to it. The fshared argument is no an integer. I left some of these comments along as they are simply removed in future patches. Some of the commentary refering to futexes by virtual page mappings was not very clear, and completely accurate (as for shared futexes both the page and the offset are used to determine the key). For the purposes of the function description, just referring to "the futex" seems sufficient. With hashed futexes we now access the page after the hash-bucket is locked, and not only after it is enqueued. When setting up the first chunk, arch can ask to set aside certain number of bytes right after the core static area which is available only through a separate reserved allocator. This will be used primarily for module static percpu variables on architectures with limited relocation range to ensure that the module perpcu symbols are inside the relocatable range. If reserved area is requested, the first chunk becomes reserved and isn't available for regular allocation. If the first chunk also includes piggy-back dynamic allocation area, a separate chunk mapping the same region is created to serve dynamic allocation. The first one is called static first chunk and the second dynamic first chunk. Although they share the page map, their different area map initializations guarantee they serve disjoint areas according to their purposes. This is the second half of the changes to make the irq_desc_ptrs be variable sized based on nr_cpu_ids. This is done by adding a new "max_nr_irqs" macro to irq_vectors.h (and a dummy in irqnr.h) to return a max NR_IRQS value based on NR_CPUS or nr_cpu_ids. This necessitated moving the define of MAX_IO_APICS to a separate file (asm/apicnum.h) so it could be included without the baggage of the other asm/apicdef.h declarations.
