Now, we start converting tty buffer functions to actually use
tty_port. This will allow us to get rid of the need of tty in many
call sites. Only tty_port will needed and hence no more
tty_port_tty_get in those paths.
tty_insert_flip_char is the next one to proceed. This one is used all
over the code, so the patch is huge.

Commit bdb498c20040 "TTY: hvc_console, add tty install" took the port
refcounting out of hvc_open()/hvc_close(), but failed to remove the
kref_put() and tty_kref_put() calls in hvc_hangup() that were there to
remove the extra references that hvc_open() had taken.
The result was that doing a vhangup() when the current terminal was
a hvc_console, then closing the current terminal, would end up calling
destroy_hvc_struct() and making the port disappear entirely. This
meant that Fedora 17 systems would boot up but then not display the
login prompt on the console, and attempts to open /dev/hvc0 would
give a "No such device" error.
This fixes it by removing the extra kref_put() and tty_kref_put() calls.

After commit "TTY: move tty buffers to tty_port", the tty buffers are
not freed in some drivers. This is because tty_port_destructor is not
called whenever a tty_port is freed. This was an assumption I counted
with but was unfortunately untrue. So fix the drivers to fulfil this
assumption.
To be sure, the TTY buffers (and later some stuff) are gone along with
the tty_port, we have to call tty_port_destroy at tear-down places.
This is mostly where the structure containing a tty_port is freed.
This patch does exactly that -- put tty_port_destroy at those places.

Pull Xen fixes from Konrad Rzeszutek Wilk:
"This has four bug-fixes and one tiny feature that I forgot to put
initially in my tree due to oversight.
The feature is for kdump kernels to speed up the /proc/vmcore reading.
There is a ram_is_pfn helper function that the different platforms can
register for. We are now doing that.
The bug-fixes cover some embarrassing struct pv_cpu_ops variables
being set to NULL on Xen (but not baremetal). We had a similar issue
in the past with {write|read}_msr_safe and this fills the three
missing ones. The other bug-fix is to make the console output (hvc)
be capable of dealing with misbehaving backends and not fall flat on
its face. Lastly, a quirk for older XenBus implementations that came
with an ancient v3.4 hypervisor (so RHEL5 based) - reading of certain
non-existent attributes just hangs the guest during bootup - so we
take precaution of not doing that on such older installations.
Feature:
- Register a pfn_is_ram helper to speed up reading of /proc/vmcore.
Bug-fixes:
- Three pvops call for Xen were undefined causing BUG_ONs.
- Add a quirk so that the shutdown watches (used by kdump) are not
used with older Xen (3.4).
- Fix ungraceful state transition for the HVC console."

Backend drivers shouldn't transistion to CLOSED unless the frontend is
CLOSED. If a backend does transition to CLOSED too soon then the
frontend may not see the CLOSING state and will not properly shutdown.
So, treat an unexpected backend CLOSED state the same as CLOSING.

The pseries hvterm driver only registers a udbg backend (for xmon and
other low level debugging mechanisms) when hvc0 is recognized as the
firmware console at boot time, not if it's detected later on, for
example because the firmware is using a graphics card.
This can make debugging challenging especially under X11, and there's
really no good reason for that limitation, so let's hookup udbg
whenever hvc0 is detected instead.

hvc_console has two methods to instanciate the consoles.
hvc_instanciate is meant to be called at early boot, while hvc_alloc is
called for more dynamically probed objects.
Currently, it only deals with adding kernel consoles in the former case,
which means for example that if a console only uses dynamic probing, it
will never be usable as a kernel console even when specifying
console=hvc0 explicitly, which could be considered annoying...
More specifically, on pseries, we only do the early instanciate for the
console currently used by the firmware, so if you have your firmware
configured to go to a video card, for example, you cannot get your
kernel console, oops messages, etc... on your serial port or hypervisor
console, which would be handy to deal with oopses.
This fixes it by checking if hvc_console.flags & CON_ENABLED is set when
registering a new dynamic console, and if not, redo the index check and
re-register the console if the index matches, allowing console=hvcN to
work.

Pull workqueue changes from Tejun Heo:
"This is workqueue updates for v3.7-rc1. A lot of activities this
round including considerable API and behavior cleanups.
* delayed_work combines a timer and a work item. The handling of the
timer part has always been a bit clunky leading to confusing
cancelation API with weird corner-case behaviors. delayed_work is
updated to use new IRQ safe timer and cancelation now works as
expected.
* Another deficiency of delayed_work was lack of the counterpart of
mod_timer() which led to cancel+queue combinations or open-coded
timer+work usages. mod_delayed_work[_on]() are added.
These two delayed_work changes make delayed_work provide interface
and behave like timer which is executed with process context.
* A work item could be executed concurrently on multiple CPUs, which
is rather unintuitive and made flush_work() behavior confusing and
half-broken under certain circumstances. This problem doesn't
exist for non-reentrant workqueues. While non-reentrancy check
isn't free, the overhead is incurred only when a work item bounces
across different CPUs and even in simulated pathological scenario
the overhead isn't too high.
All workqueues are made non-reentrant. This removes the
distinction between flush_[delayed_]work() and
flush_[delayed_]_work_sync(). The former is now as strong as the
latter and the specified work item is guaranteed to have finished
execution of any previous queueing on return.
* In addition to the various bug fixes, Lai redid and simplified CPU
hotplug handling significantly.
* Joonsoo introduced system_highpri_wq and used it during CPU
hotplug.
There are two merge commits - one to pull in IRQ safe timer from
tip/timers/core and the other to pull in CPU hotplug fixes from
wq/for-3.6-fixes as Lai's hotplug restructuring depended on them."
Fixed a number of trivial conflicts, but the more interesting conflicts
were silent ones where the deprecated interfaces had been used by new
code in the merge window, and thus didn't cause any real data conflicts.

flush[_delayed]_work_sync() are now spurious. Mark them deprecated
and convert all users to flush[_delayed]_work().
If you're cc'd and wondering what's going on: Now all workqueues are
non-reentrant and the regular flushes guarantee that the work item is
not pending or running on any CPU on return, so there's no reason to
use the sync flushes at all and they're going away.
This patch doesn't make any functional difference.

So now for those drivers that can use neither tty_port_install nor
tty_port_register_driver but still have tty_port available before
tty_register_driver we use newly added tty_port_link_device.
The rest of the drivers that still do not provide tty_struct <->
tty_port link will have to be converted to implement
tty->ops->install.

alternative would be to keep points and refcount the termios objects.

1. They are tiny anyway

Pull five Xen bug-fixes from Konrad Rzeszutek Wilk:
- When booting as PVHVM we would try to use PV console - but would not validate
the parameters causing us to crash during restore b/c we re-use the wrong event
channel.
- When booting on machines with SR-IOV PCI bridge we didn't check for the bridge
and tried to use it.
- Under AMD machines would advertise the APERFMPERF resulting in needless amount
of MSRs from the guest.
- A global value (xen_released_pages) was not subtracted at bootup when pages
were added back in. This resulted in the balloon worker having the wrong
account of how many pages were truly released.
- Fix dead-lock when xen-blkfront is run in the same domain as xen-blkback.

We need to make sure that those parameters are setup to be correct.
As such the value of 0 is deemed invalid and we find that we
bail out. The hypervisor sets by default all of them to be zero
and when the hypercall is done does a simple:
a.value = d->arch.hvm_domain.params[a.index];
Which means that if the Xen toolstack forgot to setup the proper
HVM_PARAM_CONSOLE_EVTCHN (or the PFN one), we would get the
default value of 0 and use that.

A -next commit "TTY: HVC, use tty from tty_port" switched the driver
to use tty_port helper for tty refcounting. But it omitted to remove
manual tty refcounting from open, close and hangup. So now we are

- use tty, not hp->tty wherever possible
- pass tty down to some functions and go to step one
- do not defer tty_hangup calls -- it is as simple as schedule_work,
so might be called with hp->lock held
- do not defer tty buffer flips -- since the driver does not use
low_latency (it cannot actually), the flip is a simple tail move
plus schedule_work. It will make our life easier in the next patch.

It is in termios cflags. So change the test in hvsi_recv_control to do
the right thing. Previously it was actually testing TTY_LDISC_OPEN
bit, i.e. whether an ldisc is active. And yes, it is most of the time.

No refcounting, just a switch. The locking in the driver prevents
races, so in fact the refcounting is not needed. But while we have a
tty in tty_port, don't duplicate that and remove the one from
hvcs_struct.

Now, count is used from tty_port and protected by tty_port->lock.
n_outbuf is left unprotected in hvc_hangup now, because there is no
