DMA_VIRT_OPS requires that dma_addr_t is at least as wide as a
pointer, which is expressed as a dependency on !64BIT ||
ARCH_DMA_ADDR_T_64BIT.
For parisc64 this is not true, and if these IB modules are enabled,
kconfig warns:
WARNING: unmet direct dependencies detected for DMA_VIRT_OPS
Depends on [n]: HAS_DMA [=y] && (!64BIT [=y] || ARCH_DMA_ADDR_T_64BIT)
Selected by [m]:
- INFINIBAND_RDMAVT [=m] && INFINIBAND [=m] && 64BIT [=y] && PCI [=y]
- RDMA_RXE [=m] && INET [=y] && PCI [=y] && INFINIBAND [=m]
Add dependencies to fix this.

No reason to have dependency on PCI for the entire infiniband stack so
move it to KConfig of only the drivers that actually using PCI.

Make the rxe and rdmavt drivers use dma_virt_ops. Update the
comments that refer to the source files removed by this patch.
Remove struct ib_dma_mapping_ops. Remove ib_device.dma_ops.

There is a strict policy in the Linux kernel that new drivers must be
disabled by default. Hence leave out the "default m" line from Kconfig.

This patch introduces the basics for a new module called rdma_vt. This new
driver is a software implementation of the InfiniBand verbs and aims to
replace the multiple implementations that exist and duplicate each others'
code.
While the call to actually register the device with the IB core happens in
rdma_vt, most of the work is still done in the drivers themselves. This
will be changing in a follow on patch this is just laying the groundwork
for this infrastructure.
