With calls to modular infrastructure, these files really
needs the full module.h header. Call it out so some of the
cleanups of implicit and unrequired includes elsewhere can be
cleaned up.

These files were getting <linux/module.h> via an implicit non-obvious
path, but we want to crush those out of existence since they cost
time during compiles of processing thousands of lines of headers
for no reason. Give them the lightweight header that just contains
the EXPORT_SYMBOL infrastructure.

The file rcutiny.c does not need moduleparam.h header, as
there are no modparams in this file.
However rcutiny_plugin.h does define a module_init() and
a module_exit() and it uses the various MODULE_ macros, so
it really does need module.h included.

Through various other implicit include paths, some files were
getting the full module.h file, and hence living the illusion
that they really only needed moduleparam.h -- but the reality
is that once you remove the module.h presence, these show up:
kernel/params.c:583: warning: ‘struct module_kobject’ declared inside parameter list
Such files really require module.h so simply make it so. As the
file module.h grabs moduleparam.h on the fly, all will be well.

With the module.h usage cleanup, we'll get this:

Up until now, this file was getting percpu.h because nearly every
file was implicitly getting module.h (and all its sub-includes).
But we want to clean that up, so call out percpu.h explicitly.
Otherwise we'll get things like this on an ARM build:
kernel/irq_work.c:48: error: expected declaration specifiers or '...' before 'irq_work_list'
kernel/irq_work.c:48: warning: type defaults to 'int' in declaration of 'DEFINE_PER_CPU'
The same thing was happening for builds on ARM for asm/processor.h
kernel/irq_work.c: In function 'irq_work_sync':
kernel/irq_work.c:166: error: implicit declaration of function 'cpu_relax'

These files were implicitly relying on <linux/kmod.h> coming in via
module.h, as without it we get things like:
kernel/power/suspend.c:100: error: implicit declaration of function ‘usermodehelper_disable’
kernel/power/suspend.c:109: error: implicit declaration of function ‘usermodehelper_enable’
kernel/power/user.c:254: error: implicit declaration of function ‘usermodehelper_disable’

These files are doing things like module_put and try_module_get

rather than getting it via <linux/device.h> which we ideally want
to remove the module.h inclusion from.

The changed files were only including linux/module.h for the
EXPORT_SYMBOL infrastructure, and nothing else. Revector them
onto the isolated export header for faster compile times.
Nothing to see here but a whole lot of instances of:
-#include <linux/module.h>
+#include <linux/export.h>
This commit is only changing the kernel dir; next targets
will probably be mm, fs, the arch dirs, etc.

There are files which use module_param and MODULE_PARM_DESC
back to back. They only include moduleparam.h which makes sense,
but the implicit presence of module.h everywhere hid the fact
that MODULE_PARM_DESC wasn't in moduleparam.h at all. Relocate
the macro to moduleparam.h so that the moduleparam infrastructure
can be used independently of module.h

A lot of files pull in module.h when all they are really
looking for is the basic EXPORT_SYMBOL functionality. The
recent data from Ingo[1] shows that this is one of several
instances that has a significant impact on compile times,
and it should be targeted for factoring out (as done here).
Note that several commonly used header files in include/*
directly include <linux/module.h> themselves (some 34 of them!)
The most commonly used ones of these will have to be made
independent of module.h before the full benefit of this change
can be realized.
We also transition THIS_MODULE from module.h to export.h,
since there are lots of files with subsystem structs that
in turn will have a struct module *owner and only be doing:
.owner = THIS_MODULE;
and absolutely nothing else modular. So, we also want to have
the THIS_MODULE definition present in the lightweight header.

Always failing to register I2C buses when the line testing fails is a
little harsh. While such a failure is definitely a bug in the driver
that exposes the affected I2C bus, things may still work fine if the
missing initialization steps are done later, before the I2C bus is
used. So it seems a better debugging tool to just report the test
failure by default. I introduce bit_test=2 if anyone really misses the
original behavior of bit_test=1.

With per-bus iommu_ops the iommu_found function needs to
work on a bus_type too. This patch adds a bus_type parameter
to that function and converts all call-places.
The function is also renamed to iommu_present because the
function now checks if an iommu is present for a given bus
and does not check for a global iommu anymore.

Summary:
Users of the pci_dma_sync_single_* api allow users to sync address ranges within
the range of a mapped entry (i.e. you can dma map address X to dma_addr_t A and
then pci_dma_sync_single on dma_addr_t A+1. The dma-debug library however
assume dma syncs will always occur using the base address of a mapped region,
and uses that assumption to find entries in its hash table. Since thats often
(but not always the case), the dma debug library can give us false errors about
missing entries, which are reported as syncing of memory not allocated by the
driver. This was noted in the cxgb3 driver as this error:

Make report_iommu_fault() return -ENOSYS whenever an iommu fault
handler isn't installed, so IOMMU drivers can then do their own
platform-specific default behavior if they wanted.
Fault handlers can still return -ENOSYS in case they want to elicit the
default behavior of the IOMMU drivers.

commit 4f3f8d9 "iommu/core: Add fault reporting mechanism" added
the public iommu_set_fault_handler() symbol but forgot to export it.
Fix that.

Start using the generic fault report mechanism, as provided by
the IOMMU core, and remove its now-redundant omap_iommu_set_isr API.
Currently we're only interested in letting upper layers know about the
fault, so in case the faulting device is a remote processor, they could
restart it.
Dynamic PTE/TLB loading is not supported.

Add iommu fault report mechanism to the IOMMU API, so implementations
could report about mmu faults (translation errors, hardware errors,
etc..).
Fault reports can be used in several ways:
- mere logging
- reset the device that accessed the faulting address (may be necessary
in case the device is a remote processor for example)
- implement dynamic PTE/TLB loading
A dedicated iommu_set_fault_handler() API has been added to allow
users, who are interested to receive such reports, to provide
their handler.

The iommu module on omap contains a few functions that are
only used by the debug module. These are however only there
when the debug code is built as a module. Since it is possible
to build the debug code into the kernel, the functions should
also be provided for the built-in case.

Without this patch it is possible to select the VIDEO_OMAP3
driver which then selects OMAP_IOVMM. But the omap iommu
driver is not compiled without IOMMU_SUPPORT enabled. Fix
that by forcing OMAP_IOMMU and OMAP_IOVMM are enabled before
VIDEO_OMAP3 can be selected.

Users of the IOMMU API (kvm specifically) assume that iommu_unmap()
returns the order of the unmapped page.
Fix omap_iommu_unmap() to do so and adopt omap-iovmm accordingly.

omap_iovmm requires page-aligned buffers, and that sometimes causes
omap3isp failures (i.e. whenever the buffer passed from userspace is not
page-aligned).
Remove this limitation by rounding the address of the first page entry
down, and adding the offset back to the device address.
