In preparation for consolidating all I/O request state, get rid of the
type field from struct hv_storvsc_request and instead use the
equivalent state in struct vmscsi_request - data_in field.
In the current code there is a call to zero out the struct vstor_packet in
stor_vsc_on_io_request(). So, to be able to directly set the state in the
vstor_packet in blkvsc_drv.c and storvsc_drv.c, get rid of the call to
zero out the packet in stor_vsc_on_io_request() and instead allocate the
request structure that has been zeroed out.

Now that vmbus_driver is a pci driver,
cleanup the irq allocation mess by using the standard
irq allocation mechanisms.
Note that this patch generates an error when the checkpatch
script is run because of the IRQF_SAMPLE_RANDOM flag used in
request_irq() function. This interrupt may be the only
external event this VM will get and consequently if this
flag (IRQF_SAMPLE_RANDOM) is not specified, experimentally
we have shown that the entropy in the VM will very very low.

Now we can complete the cleanup of the root device
management - use the pci device as the root device for
all Hyper-V devices. As part of this cleanup get rid
of the root device object from vmbus_driver_context.

The power-mode sequence on MMC is MMC_POWER_OFF -> MMC_POWER_UP ->
MMC_POWER_ON and not MMC_POWER_ON -> MMC_POWER_UP, as the driver currently

SDIO IRQs got unmasked on sh-mobile while writing to the
CTL_CLK_AND_WAIT_CTL register, because that register at address 0x138
is not implemented on those SoCs and writes to it overwrite the
register at address 0x38: CTL_SDIO_IRQ_MASK. Previous patches
eliminated access to register above 0xff on sh-mobile, so that this
work-around isn't needed anymore.

Not all tmio implementations have registers above oxff. Accessing
them on thise platforms is dangerous. In some cases it leads to
address wrapping to addresses below 0x100, which corrupts random
unrelated registers.

On sh-mobile platforms the SDHI driver was using the tmio_mmc SD/SDIO
MFD cell driver. Now that the tmio_mmc driver has been split into a
core and a separate MFD glue, we can support SDHI natively without the
need to emulate an MFD controller. This also allows to support systems
with an on-SoC SDHI controller and a separate MFD with a TMIO core.

The CONFIG_MFD_SH_MOBILE_SDHI Kconfig symbol is going to disappear soon,
switch ecovec to using CONFIG_MMC_TMIO(_MODULE).
