The patchset provides two new functions in mm/slab.h and modifies SLAB
and SLUB to use these. The kmem_cache_node structure is shared between
both allocators and the use of common accessors will allow us to move
more code into slab_common.c in the future.
This patch (of 3):
These functions allow to eliminate repeatedly used code in both SLAB and
SLUB and also allow for the insertion of debugging code that may be
needed in the development process.

When umount is issued during recovery on the new master that has not
finished remastering locks, it triggers BUG() in
dlm_send_mig_lockres_msg(). Here is the situation:
1) node A has a lock on resource X mastered by node B.
2) node B dies -> node A sets recovering flag for res X
3) Node C becomes the new master for resources owned by the
dead node and is remastering locks of the dead node but
has not finished the remastering process yet.
4) umount is issued on node C.
5) During processing of umount, ignoring unfished recovery,
node C attempts to migrate resource X to node A.
6) node A finds res X in DLM_LOCK_RES_RECOVERING state, considers
it a logic error and sends back -EFAULT.
7) node C asserts BUG() upon seeing EFAULT resp from node B.
Fix is to delay migrating res X till remastering is finished at which
point recovering flag will be cleared on both A and C.

ocfs2_search_extent_list may return -1, so we should check the return
value in ocfs2_split_and_insert, otherwise it may cause array index out of
bound.
And ocfs2_search_extent_list can only return value less than
el->l_next_free_rec, so check if it is equal or larger than
le16_to_cpu(el->l_next_free_rec) is meaningless.

Commit 85816794240b ("fanotify: Fix use after free for permission
events") introduced a double free issue for permission events which are
pending in group's notification queue while group is being destroyed.
These events are freed from fanotify_handle_event() but they are not
removed from groups notification queue and thus they get freed again
from fsnotify_flush_notify().
Fix the problem by removing permission events from notification queue
before freeing them if we skip processing access response. Also expand
comments in fanotify_release() to explain group shutdown in detail.

Rename fsnotify_add_notify_event() to fsnotify_add_event() since the
"notify" part is duplicit. Rename fsnotify_remove_notify_event() and
fsnotify_peek_notify_event() to fsnotify_remove_first_event() and
fsnotify_peek_first_event() respectively since "notify" part is duplicit
and they really look at the first event in the queue.

This commit is a guesswork, but it seems to make sense to drop this
break, as otherwise the following line is never executed and becomes
dead code. And that following line actually saves the result of
local calculation by the pointer given in function argument. So the
proposed change makes sense if this code in the whole makes sense (but I
am unable to analyze it in the whole).

The LDC handshake could have been asynchronously triggered
after ldc_bind() enables the ldc_rx() receive interrupt-handler
(and thus intercepts incoming control packets)
and before vio_port_up() calls ldc_connect(). If that is the case,
ldc_connect() should return 0 and let the state-machine
progress.

Based almost entirely upon a patch by Christopher Alexander Tobias
Schulze.
In commit db64fe02258f1507e13fe5212a989922323685ce ("mm: rewrite vmap
layer") lazy VMAP tlb flushing was added to the vmalloc layer. This
causes problems on sparc64.
Sparc64 has two VMAP mapped regions and they are not contiguous with
eachother. First we have the malloc mapping area, then another
unrelated region, then the vmalloc region.
This "another unrelated region" is where the firmware is mapped.
If the lazy TLB flushing logic in the vmalloc code triggers after
we've had both a module unload and a vfree or similar, it will pass an
address range that goes from somewhere inside the malloc region to
somewhere inside the vmalloc region, and thus covering the
openfirmware area entirely.
The sparc64 kernel learns about openfirmware's dynamic mappings in
this region early in the boot, and then services TLB misses in this
area. But openfirmware has some locked TLB entries which are not
mentioned in those dynamic mappings and we should thus not disturb
them.
These huge lazy TLB flush ranges causes those openfirmware locked TLB
entries to be removed, resulting in all kinds of problems including
hard hangs and crashes during reboot/reset.
Besides causing problems like this, such huge TLB flush ranges are
also incredibly inefficient. A plea has been made with the author of
the VMAP lazy TLB flushing code, but for now we'll put a safety guard
into our flush_tlb_kernel_range() implementation.
Since the implementation has become non-trivial, stop defining it as a
macro and instead make it a function in a C source file.

Fix detection of BREAK on sunsab serial console: BREAK detection was only
performed when there were also serial characters received simultaneously.
To handle all BREAKs correctly, the check for BREAK and the corresponding
call to uart_handle_break() must also be done if count == 0, therefore
duplicate this code fragment and pull it out of the loop over the received
characters.
Patch applies to 3.16-rc6.

Fix regression in bbc i2c temperature and fan control on some Sun systems
that causes the driver to refuse to load due to the bbc_i2c_bussel resource not
being present on the (second) i2c bus where the temperature sensors and fan
control are located. (The check for the number of resources was removed when
the driver was ported to a pure OF driver in mid 2008.)

The assumption was that update_mmu_cache() (and the equivalent for PMDs) would
only be called when the PTE being installed will be accessible by the user.
This is not true for code paths originating from remove_migration_pte().
There are dire consequences for placing a non-valid PTE into the TSB. The TLB
miss frramework assumes thatwhen a TSB entry matches we can just load it into
the TLB and return from the TLB miss trap.
So if a non-valid PTE is in there, we will deadlock taking the TLB miss over
and over, never satisfying the miss.
Just exit early from update_mmu_cache() and friends in this situation.

sparc64 io refactoring
The following patchset refactor io_64.h. This was triggered by
another patchset by Thierry Reding that updates the the generic
io.h such that it may be used by sparc64 and thus make sparc
and sparc64 more equal in this area.
Before attempting to introduce the generic version it was
necessary to clean up the current state to avoid any mistakes.
The updates from Thierry needs to go in before I will attempt
introducing the generic io.h.
The sparcspkr used inb()/outb() primitives
for of_ioremap() memory. Update this driver to use sbus_ variants.
This change was triggered by a number of new warnings that would otherwise
have been seen when dropping the macro indirections.
Likewise PeeCeeI.c had some warnings that was fixed by
using another IO varaint - which simplified the code too.
The patchs has been generated with the --histogram option
to make the patch that re-order functions in io_64.h
more readable.
So to see the same patches locally you need this option too.
I have not yet any working sparc64 HW - so this is not tested.
But I brought a SUN NetraX1 that I am working on gettting
running. That may allow me to do some minimal tests in the future.

Several of the small IO functions ended up having the same implementation.
Use __raw_{read,write}* + {read,write}* as base for the others.
Continue to use static inline functions to get full type check.
The size of vmlinux for a defconfig build was the same when
using static inline and macros for the functions - so there
was no size win when using macros.
This was tested with gcc 4.8.2 + binutils 2.24.
For such simple constructs I assume older gcc's will
do the same job.

Most likely for historical reasons io_64.h used an
extra layer of macro indirections.
Fix it so we no longer use these indirections.
In the process we loose a cast to the addr argument for in*()/out*()
but all known affected users has already been fixed so
no warnings are triggered.
For each of the IO functions add a proper define like this:
#define inb inb
This is done to make the code compatible with the way these
functions are defined in asm-generic/io.h with the objective
to later introduce the generic io.h for sparc64.

The PeeCeeI.c code used in*() + out*() for IO access.
But these are in little endian and the native (big) endian
result was required which resulted in some bit-shifting.
Shift the code over to use the __raw_*() variants all over.
This simplifies the code as we can drop the calls
to le16_to_cpu() and le32_to_cpu().
And it should be a little faster too.
With this change we now uses the same type of IO access functions
in all of the file.

The memory are mapped using of_ioremap() which is
an indication this is sbus memory.
Shift all uses of inb/outb to the sbus variants.
The inb/outb methods uses ASI_PHYS_BYPASS_EC_E_L,
whereas sbus_ variants uses ASI_PHYS_BYPASS_EC_E.
The difference is if the reads/writes are done in
native or little endian.
But for byte reads/writes there is no difference
so this does not matter for inb/outb - and this
driver only uses the byte variants.

/dev/mdesc on Linux does not support reading arbitrary number
of bytes and seeking while /dev/mdesc on Solaris does. This
causes tools that work on Solaris to break on Linux. This patch
adds these two capabilities to /dev/mdesc.

This patch moves data allocated using kzalloc to managed data allocated
using devm_kzalloc and cleans now unnecessary kfrees in probe and remove
functions. The header file is added to make the devm function explicitly
