lib/bug.c: convert printk to pr_foo()
- Coalesce formats
- "WARNING:" prefix unchanged to keep bug format.
- printk(KERN_DEFAULT not converted.
- define pr_fmt without prefix to avoid any default prefix update
(suggested by Joe Perches).
