Pull SCSI fixes from James Bottomley: "This is a set of 13 fixes, a MAINTAINERS update and a sparse update. The fixes are mostly correct value initialisations, avoiding NULL derefs and some uninitialised pointer avoidance. All the patches have been incubated in -next for a few days.
