[SCSI] Add Documentation and integrate into docbook build
Add Documentation/DocBook/scsi_midlayer.tmpl, add to Makefile, and update lots of kerneldoc comments in drivers/scsi/*. Updated with comments from Stefan Richter, Stephen M.
