Subject: Miscellaneous fixes:

  * Clear extraneous argument to the Os macro.
  * Place the name description on the Nd line.
  * Mark sub-sections up with Ss, not Sh.
  * Don't double-quote "Login" when "login prompt" is perfectly
    good English.
