vnodes. This will hopefully serve as a base from which we can
expand the MP code. We currently do not attempt to obtain any
mutex or SX locks, but the door is open to add them when we nail
down exactly how that part of it is going to work.

This will make a number of things easier in the future, as well as (finally!)
avoiding the Id-smashing problem which has plagued developers for so long.
Boy, I'm glad we're not using sup anymore. This update would have been
insane otherwise.

Protected them with `#ifdef KERNEL' so that <sys/queue.h> is valid C++.
Added the necessary #includes of <sys/queue.h>.
These functions are bogus and should be replaced by the queue macros.
