New release notes: pcvt(4) gone, pc98 bootable CDROM support

(and bootable CDROM releases), updated NTP manpages.

Modified release notes:  Munged ipfw(4) release note to remove
now-redundant remark about ip6fw going away "in future releases",
noted addition of new ext2fs(5) manpage.
