Style cleanups:
* be consistant on protecting "#define FOO"s with "#undef FOO".
* be consistant that macro params are upper case, and commas are followed
by a space in the macro definition
* protect macro param expansion by ()'s
* break long lines
* line continuations chars to consistant column
* remove trailing spaces
